Who we are

Vitalida Physio & Ostéo is the practice of Alida Farrell, physiotherapist and osteopath, inside the NDG Health Clinic, 3792 Décarie Blvd., Suite 8, Montreal, Quebec H4A 3J7. We are committed to protecting your personal information in accordance with Quebec's Act respecting the protection of personal information in the private sector (Law 25).

The information we collect

Through this site's form: your name, your email, your phone number if you give it, the reason you choose and your message. Nothing else is collected without your knowledge. Online booking is done on GOrendezvous, a separate service with its own policy.

What we do with it

Answering you, setting an appointment, preparing your first meeting. Your information is used for nothing else: no newsletter without your consent, no sale or rental of your data.

Message transmission

Messages sent through the form pass through a form service provider (Formspree) whose servers may be located outside Canada, notably in the United States. By submitting the form, you consent to this transfer. The site is hosted by Netlify and loads fonts from Google Fonts, which may record your IP address.

Cookies

This site sets one essential cookie in your browser, to remember your choices. No advertising cookies, no tracking tools.

The back care video page contains a video hosted by YouTube (Google). It loads only if you accept external content; YouTube may then set its own cookies and receive your IP address. You can change your mind at any time through the “Your cookies” link at the bottom of each page, or the button at the bottom left of the screen.

Retention and security

We keep your messages as long as needed to answer you and ensure your follow-up, then delete them securely. Access is limited to Alida Farrell.

Your rights

You may request access to your information, have it corrected or deleted, and withdraw your consent at any time by writing to vitalidaphysioosteo@gmail.com. We reply within thirty days.

Complaints

If you believe your rights are not being respected, you may contact the Commission d'accès à l'information du Québec (cai.gouv.qc.ca).

Changes

This policy may be updated. The date at the top of the page indicates the version in effect.
